Usage of a Curved Chisel When Resecting Osteochondroma in the Long Bone
نویسنده
چکیده
To the Editor: Osteochondroma, which is the most common benign bone tumor, is an exophytic bone tumor covered by a cap of cartilage. It exists in two clinical settings, as a solitary lesion (solitary osteochondroma) and as a multiple lesion (multiple hereditary exostoses). The most common affected site is the metaphysis around the knee, especially in the distal femur. When pain or neurologic symptoms develop due to compression, resection of the tumor is recommended. The resection line is supposed to be at the base of the tumor, namely at the border with the normal bone. The recurrence of osteochondroma is rare, but it does recur in an estimated 2% of cases. Moreover, the development of osteosarcoma at the initial site of a treated osteochondroma has been reported. In addition, osteosarcoma has been reported within the stalk of an osteochondroma. The most common resection tools are a flat chisel, a bone saw or a wire-saw. However, these tools make complete resection of an osteochondroma in the metaphysis of a lone bone difficult, due to continuity of the osteochondroma and the host bone, and because of the anatomical concave shape of the metaphysis of the host bone. In contrast, usage of a curved chisel (radius, 40 or 45 mm; length, 48.5 or 65 mm; width, 15 or 20 mm; Sonne Co. Tokyo, Japan) (Fig. 1) for transposition osteotomy of the acetabulum with or without an intraoperative radiograph makes complete resection of osteochondroma in the long bone easier, thereby minimizing the chance of its recurrence and reducing the possibility of the subsequent development of a malignant bone tumor (Fig. 2).

Background: Osteochondroma is a common tumor of the skeletal bone and is a common benign tumor of the bone constitutes 10-15% of all and 20-50% of the benign bone tumors. The lesion is an exophytic bony protrusion covered by a cartilaginous cap. It is most commonly found in long bones, and especially at the epiphysis. Nearly 40% of cases are seen around the knee joint. Osteochondroma rarel...
متن کاملExtensive Osteochondroma of Talus Presenting as Tarsal Tunnel Syndrome: Report of a case and Literature Review
Osteochondroma or exostosis is the most common benign bone tumor, and occurring frequently in the proximal humerus, tibia, and distal femur. It rarely affects talus. Osteochondroma of talus is a very rare etiology of tarsal tunnel syndrome (TTS). We report a rare case of extensive osteochondroma of the talus in a 60 year old female presenting with multiple swellings around the ankle and sympt...
متن کاملComputer-assisted spherical osteotomy with a curved-bladed Tuke Saw.
Techniques for spherical osteotomy, such as rotational acetabular osteotomy, can help orthopaedic surgeons correct bony deformities and remove spherical acetabular components. Curved chisels are used during a spherical osteotomy, but they require skill and have a potential risk of damaging blood vessels or nerves. In order to perform a precise, quick and safe spherical osteotomy, we have develo...
متن کاملCase Report: Osteochondroma of the mandible
Osteochondroma is an osseous protuberance with cartilaginous growth potential that usually forms at the ends of long bones such as knee, hip, shoulder and joints(1). Osteochondromas are one of the most common benign tumours of bone, approximately 35% to 50% of all benign tumors and 8% to 15% of all primary bone tumors (2). Only about 1% of these occur within the head and neck region (1). The mo...

Osteochondroma is a common benign tumor of the axial skeleton, especially in the distal metaphysis of the femur and the proximal metaphysis of the tibia, that can occur on the facial skeleton (albeit rarely). Osteochondroma is differentiated from chondroma, osteochondromatosis and osteoma. Osteochondroma shows an irregular radiopaque lesion and chondromatic area surrounded by the osteoma. When ...
